PDAO ID Application Requirements (New Application)
• Duly Accomplished Application Form.
• Must be a Filipino Citizen and a resident of Iloilo City
• Medical Certificate, indicating the type of Disability, issued within the last 6 months. with Doctor’s Name; Doctor’s License No.;
-Visual Disability Medical Certificate should be issued by an OPTHALMOLOGIST
• Two (2) Government issued Valid IDs with Date of Birth; Birth Certificate for children below 8 years old with parents Valid ID.
•Marriage Certificate for female married Applicants.
•Barangay Certificate of Residency
• 2×2 colored picture with white background taken with the last 3 months.
• If filed thru representative, must be a next of kin, submit a letter of authority and valid ID of representative.
• Original and Photocopy of all documents
PDAO ID Application Requirements (Renewal)
• Medical Certificate, indicating the type of Disability, issued within the last 6 months with Doctor’s Name; Doctor’s License No.;
• Two (2) Government issued Valid IDs with Date of Birth; Birth Certificate for children below 8 years old with parents Valid ID.
• Marriage Certificate for female married Applicants.
• Barangay Certificate of Residency
• 2×2 colored picture with white background taken with the last 3 months.
• If filed thru representative, must be a next of kin, submit a letter of authority and valid ID of representative.
• Original and Photocopy of all documents
Replacement Requirements of PDAO ID and Purchase Booklets (Lost, Damaged, Change of Name due to Marriage)
*Note: If IDs were issued in 2022, their holders must renew them.
• Notarized Affidavit of Loss
• Marriage Certificate
• 2×2 colored picture with white background taken with the last 3 months.
• If filed thru representative, must be a next of kin, submit a letter of authority and valid ID of representative.
